Where is your office located?

Is there parking?

Yes, there is parking in the rear of the building.

Since your office is on the second floor, is there an elevator?

No, there is no elevator access to my office; currently, it is not wheelchair accessible.

What are your office hours?

You can reach Cassie Peters, my office manager, from 8:00 a.m. to 6:00 p.m., Monday through Friday.

Her direct number is (801) 620-0230. If you leave me a message, I will return the call within 24 hours.

I offer day and evening appointment availability on Mondays, Tuesdays, Wednesdays. Thursdays I offer morning appointments.

What are the age ranges of the clients you treat?

I work with ages 10 and above and treat a myriad of diagnoses including mood disorders, anxiety, trauma, OCD, etc.

Do you work with families, couples, and groups?

All the above.

Do you accept insurance?

I accept Select Health, Blue Cross Blue Shield, Educators Mutual, Optum/United Health, and Aetna.

What is the amount if I private pay?

I charge $125 per session. Sessions last 60 minutes.

Do you have a sliding scale?

Currently, I do not have a sliding scale payment schedule.

What is your cancellation policy?

If the session is not cancelled with a 24-hour notice, I charge half of the session cost.

Do you offer a free consultation?

I do a free telephone consultation, so you can see if I am the right fit for you. Alternatively, you can simply schedule an appointment.

Do you work with people from different religious backgrounds and faiths?

Yes, I do. I work with people from all walks of life and faiths.

What can I expect with my first session?

You will be sent a packet to fill out prior to your first visit. Please bring it with you when you come for the first visit.

If it’s complete, it will be reviewed in the first session. You will be able to give details regarding your answers. Otherwise, we will be completing the paperwork during the session.

How long will I be in therapy?

It depends on the presenting issues and how debilitating the issues have become.

However, my job is to put myself out of a job. I don’t keep people in therapy longer than they need, and you always have the option of ending therapy at any time.

Do you do any public speaking engagements?

Yes, with enough notice, I do public speaking engagements.
